During your visit to TRIO, you may be asked to fill out a form in regards to your treatments with us. All information collected with be via an electronic device and will be stored against your name. Any information provided will not be shared, sold or accessed by anyone other than your therapists and will be available for you to see at any time.
Most browsers are initially set up to accept cookies; however, you can reset your browser to refuse all cookies or indicate when a cookie is being sent or you can flush your browser of cookies from time to time (Note: you may need to consult the help area of your browser application for instructions).
